This survey is designed to give you and your colleagues a “snapshot” of how you score the four factors; Goals, Roles, Interpersonal Relations, and Procedures for your organization. Each person is asked to complete the survey and return the completed survey per the instructions given by the person coordinating the effort. You are asked to read the statements at the ends of each line and then circle the number that corresponds to how you feel about that statement. After completing the entire document, please go back and calculate the score on each sheet. When all completed surveys have been collected, various data can be tabulated and reported. The average scores as well as the range of scores on the four factors are useful benchmarks for reference points and for measuring future change. We will tabulate your totals , graph the four factors and offer some initial observations about your team's responses. You will be able to use the "snapshot" to consider different approaches to improving your performance as well as a means of measuring change among team members over a period of time by re-surveying your team members at intervals.
